How can I transfer an Kindle book from my MacBook to my iPhone? I have Kindle for Mac on both, but I can't see an option to sync ebooks.

I wonder if you do it from the Kindle section of your Amazon account? Register your iPhone as a Kindle device within your account there, select the ebook, then select "send to iPhone" from there?
A wild guess only ...

You don't. You can only transfer books you have bought via Amazon and you do this from your Kindle app. Purchased books will appear in Archived items.
Or is this a book you have imported yourself?

It's a book that I bought from Amazon and downloaded to the MacBook using Kindle. I've done what Carrie suggested, but thought that there must be a way to sync ebooks between the two.

You could use to convert them to .epub (iPhone iBook format).

If the book was bought on Amazon, you need to make sure your kindle accounts are updated to show 'kindle for mac' and 'kindle for iPhone' and then they will automatically stay in sync using Amazon Whispersync.
