3.39GB suggests a full iOS download. 99MB is definitely a 'patch' (for want of a better word). Maybe iTunes cannot handle patch updates?

I have no idea as to the trustworthiness of this site but appears to have the v12.4 ISPW files by device. They are (mostly) of the 3+GB size as mentioned. It confirms to me that they are indeed device specific.
The main takeaway for 12.4.1 is that you can't jailbreak any more. Not an idea I have myself ever entertained.

Each time that I update an iOS device, using iTunes as I have said, the download is in the 3-4 GB range. After the installation is complete, I find a similarly-sized ISPW file in the trash. Actually, it lands in the trash even before the installation is done.
This time, my phone's ISPW file landed in the trash even before I connected it! It happened while I was updating the first of my iPads. Strange. As in very.

My 5S has just updated to 12.5.1 Surprised they are still updating iOS 12

Don't ask me (I know it was rhetorical). They can try it and find out I suppose. I think I've only used FT twice in seven years so I'm unfamiliar with it. I'm a Skype user because that is cross platform.

Just checked and my 5S is running 12.5.2 but doesn’t offer an upgrade...

Nothing was showing for my 5S till I went to software update. It’s running 12.5.3 now.
